The NeuroThera Effectiveness and Safety Trial-1 evaluated the safety and efficacy of LLLT at 808 nm to enhance 90-day outcomes for ischemic stroke when administered within 24 hours of stroke onset. This study involved 120 patients with National Institutes of Health Stroke Scale (NIHSS) scores ranging from 7 to 22, excluding those receiving thrombolytic therapy. The primary outcome was defined as a complete recovery at day 90 (NIHSS score of 0–1) or a reduction in NIHSS score by at least nine points. The median time to treatment was 18 hours. Among the 79 patients treated with LLLT, 38% achieved both a final NIHSS score of 0–1 and improved by more than nine points, 20% improved by more than nine points alone, 11% reached a final score of 0–1 without a nine-point improvement, and 30% did not meet either endpoint. In the 41 control patients, the corresponding figures were 29%, 7%, 15%, and 49%, respectively. Mortality rates and adverse effects were comparable between the treated and control groups.



However, a larger follow-up multicenter double-blind study with 660 patients randomized to either LLLT or sham treatment found no significant difference in favorable outcomes, though there were favorable trends in the LLLT group regarding reduced disability at 90 days. This second study suggested that LLLT might benefit patients with lower baseline NIHSS scores.



These studies collectively provide in vivo evidence that LLLT’s effects on cytochrome oxidase and nitric oxide significantly contribute to its neuroprotective action against ischemia and potentially other metabolic insults, as seen in the benefits of LLLT after traumatic brain injury in both animals and humans. Ongoing clinical trials are exploring the advantages of transcranial LLLT in rehabilitating hemiplegic patients post-ischemic stroke. Continued clinical research is anticipated to further elucidate the effects of LLLT on stroke and neurotrauma patients.
